There are typically no other costs for PSS Surveys within the USA.
You will receive150 Survey Envelopes . Additional Survey Envelopes
may be purchased and evaluated at a cost of $17.00 per unit. Note:
Increasing the number of Survey Envelopes will create percentage
changes to group assignments. Generally, this is not recommended!
However, the choice to send out additional surveys is entirely yours.

Completion of PSS Interview/Questionnaire
Upon receipt of your Enrollment Form and Fee, a Questionnaire/Data
Form will be sent to you for completion. Once we receive the Questionnaire/Data
Form, you will be contacted to conduct an Interview with PSS.
These steps are designed to assist us in understanding your short-term
and long-term goals. Following the completion of the Interview,
all information you want to obtain from the PSS Survey will be
confirmed.
